Medical Procedure Test Preparation

To help our patients understand our test preparation procedures, we have outlined the steps below that must be followed for a successful test outcome.

Please keep in mind, if you do not follow the steps listed, your test may have to be rescheduled.
Use our Quick Links in the sidebar for details instructions on your specific test preparation procedure.

Ultrasound

  • Abdomen – Nothing to eat or drink after midnight, you may take your medication with water.
  • Abdomen/Pelvis – Nothing to eat or drink after midnight, you may take your medication and drink plenty of fluids because your bladder MUST be extremely full.
  • Pelvis – Drink plenty of fluids because your bladder MUST be extremely full.
  • Aorta – Nothing to eat or drink after midnight.

Magnetic Resonance Imaging (MRI)

  • Please inform the receptionist if you have a pacemaker. (Absolutely no Pacemakers are allowed!)
  • Please do not wear or bring any metal with you, it is not allowed in the MRI room.
  • Complete the attached document and bring it back with you to your appointment.
  • Please arrive at least 15 minutes prior to your appointment time.

Your MRI scan will be performed here at the office, please enter through the front and sign in with a receptionist.

CT Scan

IF YOU HAVE ANY ALLERGIES OR ARE PREGNANT TELL THE PHYSICIAN AND THE TECHNOLOGIST

For an abdomen and/or pelvis CT Scan, you will need to eat a LIGHT supper the night before your test. You will be given 3 bottles of contrast to drink to coat your insides.; Please drink the bottles of contrast at the following times.
  1.  Drink one bottle the night before your test after your LIGHT meal.
  2. You may NOT have any breakfast on the morning of your appointment.
  3. You may take any medication you need with water.
  4. Drink the second bottle of contrast two hours before your appointment time.
  5. Drink the third bottle one half hour before your scan is to begin.

Minor surgeries: Skin Lesion

Please come on time with at least 2 hour fasting. Please inform us if you have any allergies to anesthetics.

Medication Refill

For all medications, please contact your pharmacy to request the refills. The pharmacy will in turn fax us the request.
At Premier Medical Clinic, we always strive to provide the fastest service possible. But, please allow 48 hours for processing of your refill request. Due to high patient call volumes, the time is required to ensure proper documentation and verification of medication.
Please remember that Narcotic/Restricted medications:

  • Need to be picked up in person by the person whose name is on the prescription. The only exceptions will be patients who are disabled and can’t come in person. In that case, only one previously-designated person is allowed to pick up the prescription for him/her.
  • Are usually provided for only one month at a time. No refills.
  • Are not allowed to be sent to other states.

Form Filling Policy/Letters

If you have any form that needs to be filled or a letter than needs to be written by providers, please make an appointment to see your Provider.

Our clinic is proud to offer an open-door policy. If you are sick and need attention, don’t hesitate to call us and we will do our best to accommodate you.
We accept all sick patients. But please keep in mind that patients coming with an acute problem will be seen by the first available provider.

Referrals

Our clinic offers referrals to all types of specialties. Please see your Provider if you need a referral to a specialist.

Please allow a few days for the referrals. If you don’t hear back from us or from the specialist office in about one week, please call our office.

Billing Information.

If you have any questions about the bill you have received, our billing staff is more than willing to discuss it with you.

We do prefer that the responsible patient come in person to discuss it. But, if that is difficult, we will discuss the bill on the phone with responsible payor.

Please don’t address billing issues with the providers themselves because they will be unable to answer any billing questions.

Radiology CD Copy Request

At Premier Medical Clinic, we are proud to have fully digital imaging equipment

To request a CD of your recent Radiology Test (Xray/MRI/CT), please contact our office at (850) 584-3278 and leave a message with a receptionist.

Please allow at least 48 hours for copy to be made.

We don’t provide hard copies of our Radiology tests.

To request a copy of labs/notes or any part of your chart, please contact our office at (850) 584-3278 and leave a message with a receptionist.

We do charge nominal fee for this service.

Confidentiality Statement

Protecting the Privacy of Patients’ Health Information

INFORMATION REQUIRED TO BE PROTECTED.

  1. The privacy of all medical records and other individually identifiable health information must be protected at all times. Information relating to a patient’s health care history, diagnosis, condition, treatment, or evaluation shall be considered individually identifiable health information. Confidentiality of this health information must be maintained at all times, and may only be disclosed with the express written consent of the patient.

  2. Non-individually identifiable health information, (e.g. health information that cannot be linked to a specific patient) is not included within the definition of protected health information.

BOUNDARIES ON HEALTH INFORMATION USE AND RELEASE.

  1.  An individual’s health information can be used for health purposes only.

  2. Protect individually identifiable health information. Premier Medical Clinic shall not publish or otherwise make generally available any information or data that identifies a patient for purposes other than treatment, payment or other health care operations, without his or her express written consent. This does not restrict the internal use of such information or data that is required in the performance of the scope of work that Premier Medical Clinic has been engaged to perform for a client. Premier Medical Clinic also maintains physical, electronic, and procedural safeguards to protect individually identifiable health information. Premier Medical Clinic is always assessing those safeguards and shall make ongoing improvements to maintain and enhance our level of security for individually identifiable health information.

  3. Ensure that health information is not used for non-health purposes. Patient information can be used or disclosed only for purposes of health care treatment, payment, and operations. Health information cannot be used for purposes not related to health care without explicit authorization from the patient. For example, Premier Medical Clinic may not access the personal health information obtained by a Premier Medical Clinic affiliate for any purpose other than to perform the services, for which we were engaged, unless Premier Medical Clinic first obtains the explicit authorization of the patient.

  4. Maintain health information in a manner to protect confidentiality. All individually identifiable health information shall be maintained by Premier Medical Clinic in a confidential manner that prevents unauthorized or inadvertent disclosure to third parties. For example, Premier Medical Clinic may share confidential information with a third party under contract or affiliated with Premier Medical Clinic for the same purpose of performing the services for which we were engaged, provided that the information shall remain confidential at all times and shall be shared with only those persons that have authority to receive such information.

PENALTIES FOR MISUSE OF PERSONAL HEALTH INFORMATION

There are serious penalties for violation of the confidentiality of health information. Please be advised of the following:
  1. State Penalties. Various state laws impose criminal and civil penalties on individuals who misuse or disclose individually identifiable health information without explicit consent by the patient.

  2. Federal Penalties. HIPAA (Health Insurance Portability and Accountability Act) is a piece of federal legislation that directly addresses the protection of confidential health information. HIPAA provides for civil money penalties up to $25,000 per person, per year for violations of patient confidentiality. HIPAA also provides for federal criminal penalties.

  3. Premier Medical Clinic Penalties. Any employee who violates the privacy and confidentiality of patient health information, through disclosure or otherwise, may be subject to disciplinary action, including termination of his or her employment with Premier Medical Clinic.

Cardiac Nuclear Stress Testing

  1. Please allow 3 hours for your test.
  2. If your appointment is before noon, please come to the test without eating or drinking. (No caffeine after midnight).
  3. If your appointment is in the afternoon, please have a light meal in AM and allow 3 hours without food prior to your appointment time.
  4. If you are taking B-Blockers or Persatine as part of your medicines, please do not take a dose on the day of your test.
  5. Wear proper attire and shoes. (Ex. Comfortable walking shoes, button-up shirt, loose fitting clothes, no dresses, no boots, no T-shirts).

Hepatobiliary Scan(HIDA SCAN)

  1. Please allow 45 Minutes – 2 hours for your test.
  2. Please eat a fatty meal the night prior to your test.
  3. Please do not eat or drink anything after midnight prior to your appointment.
  4. If your appointment is in the afternoon pleas have a LIGHT meal in the AM and allow 4 – 6 hours without food or drink prior to your appointment time.

Thyroid Uptake and Scan

This is a 2 day, 3 phase study. You may eat and drink throughout the whole study.

  1. Phase 1: Plan on being here about 10-15 minutes. You will receive your radioisotope dose and return in 5-6 hours.
  2. Phase 2: Return 5-6 hours later for the uptake and scan.; Plan on being here about 1 hour.
  3. Phase 3: Return the next day, at the same time you received your dose from Phase 1 and plan on being here about 10-15 minutes.
  4. Stop all thyroid medicines 3 weeks before the study.
  5. No iodinated contrast 1 month before this study, which includes IVP, CT Scan, Heart Catherization, Etc.
  6. No antihistamines 1 week before this study which includes Cough, Sinus, Cold, Flu over the counter or prescription medicines.
  7. DO NOT take your thyroid medicines until the study is completed.
